Diagnostic paracentesis with calculation of the serum-ascites albumin gradient (SAAG) helps distinguish portal-hypertensive causes from others. Imaging (ultrasound or CT) and, where malignancy is suspected, cytology are essential. Because the causes of ascites range from manageable to life-threatening, a clear diagnosis must always precede any treatment plan, complementary or otherwise.
