Autoimmune Lymphoproliferative Syndrome (ALPS)

Autoimmune lymphoproliferative syndrome (ALPS) is a rare genetic disorder of immune regulation, most often caused by mutations in the FAS gene, that prevents the normal programmed death of excess immune cells (lymphocytes) once an immune response is no longer needed. This leads to chronic enlargement of the lymph nodes and spleen, and to autoimmune destruction of blood cells — particularly red cells, platelets, and white cells — because the immune system fails to properly regulate itself. ALPS also carries an increased long-term risk of lymphoma, making lifelong monitoring important. This is a genetic condition managed by paediatric or adult haematology and immunology specialists; homeopathy cannot correct the underlying immune-regulation gene defect but may offer general supportive care alongside that specialist management.

Understanding ALPS

Normal immune function depends on lymphocytes being produced in large numbers during an immune response and then eliminated once that response is complete, through a regulated process called apoptosis. In ALPS, mutations affecting the FAS pathway (or related genes) impair this elimination process, so lymphocytes accumulate abnormally in the lymph nodes and spleen, causing chronic, sometimes dramatic swelling of these organs from early childhood. ALPS also carries a meaningfully increased lifetime risk of lymphoma, so regular haematology follow-up, including periodic blood counts and clinical assessment of lymph node and spleen size, is a critical and ongoing part of care, alongside prompt investigation of any new, rapidly enlarging, or unusual lymph node growth.

There is no treatment that corrects the underlying FAS gene defect. Management instead focuses on controlling autoimmune flares, typically with immunosuppressive medication when active cytopenias occur, alongside regular monitoring for lymphoma risk. In severe, treatment-resistant cases, more specialised therapies may be considered by the treating haematology-immunology team. Genetic counselling is recommended for affected families, since ALPS is usually inherited in an autosomal dominant pattern with variable severity, even within the same family.
