Cervical spondylosis — age-related degenerative changes in the cervical spine including disc desiccation, osteophyte formation, facet joint arthrosis, and ligamentous thickening — is the most common cause of neck pain in adults over 40, and the most common cause of spinal cord dysfunction (myelopathy) in patients over 55. Most cervical spondylosis is asymptomatic; symptomatic disease produces neck pain and stiffness, cervical radiculopathy (arm pain, numbness, or weakness from nerve root compression), or cervical myelopathy (cord compression producing gait disturbance, hand clumsiness, and lower limb weakness). Constitutional homeopathy effectively manages neck pain and radicular symptoms in the majority of cases.

Understanding Cervical Spondylosis

Cervical disc degeneration begins in the 3rd decade, with universal radiological changes by age 60. The C5-C6 and C6-C7 levels are most commonly affected. C5-C6 spondylosis produces C6 radiculopathy — lateral arm pain, thumb and index finger paraesthesia, biceps weakness, and reduced biceps reflex. C6-C7 disease produces C7 radiculopathy — posterior arm pain, middle finger paraesthesia, triceps weakness, and reduced triceps reflex. Foraminal osteophyte encroachment compresses the exiting nerve root, while disc-osteophyte complexes encroach on the spinal cord in myelopathy.

Myelopathy — A Surgical Emergency Indicator

Cervical myelopathy — spinal cord compression producing upper motor neuron signs in the lower limbs (spasticity, hyperreflexia, Babinski sign), lower motor neuron signs in the upper limbs, and bladder dysfunction — is a progressive condition requiring neurosurgical assessment. MRI is the investigation of choice. Myelopathy does not respond to conservative management including homeopathy; untreated progressive myelopathy leads to permanent disability. Rapidly progressive myelopathy (deterioration over weeks) requires urgent neurosurgical review. Homeopathy has no role in managing established cervical myelopathy.

Constitutional Homeopathic Approach

Constitutional homeopathy for cervical spondylosis with neck pain and radiculopathy (without myelopathy) addresses the character of neck pain (stiff, burning, tearing), the radiation into the arms (side, dermatomal distribution), the triggering factors (posture, cold, turning), the thermal modalities, and the constitutional type. Treatment over 8 to 12 weeks alongside physiotherapy cervical strengthening and posture correction produces meaningful improvement in pain and radicular symptoms in most patients.
