The oral ulcers — which appear as painful, punched-out lesions on the tongue, hard palate, buccal mucosa, and gums — are often the most distressing feature for the child. They make eating, drinking, and even swallowing saliva painful, leading to drooling, refusal to feed, and dehydration risk. The skin vesicles are typically small, oval, grey, and surrounded by a red halo; unlike chickenpox vesicles, they are not particularly itchy.
HFMD is highly contagious, spreading through direct contact with nasal secretions, saliva, faeces, and fluid from the blisters. It spreads rapidly in childcare settings and schools. The illness is almost universally self-limiting in healthy children, resolving within seven to ten days, though complications — meningitis, encephalitis, and myocarditis — occur rarely and are associated primarily with EV71 strains.
When It Peaks and Who Gets It
HFMD peaks in late summer and early autumn in temperate climates, and can occur year-round in tropical and subtropical regions. It primarily affects children under five years of age, though older children and adults can be affected, particularly if they lack prior immunity to the causative strain. Nursery and playgroup-attending children are at particularly high risk due to close contact and shared surfaces.
There is no vaccine available for the most common strains circulating globally, though EV71 vaccines have been developed and are in use in some Asian countries. For most families, HFMD is an expected — if unwelcome — part of the early childhood infectious experience, and the focus of management is on symptom relief and prevention of dehydration.
Why Children Suffer and What Parents Can Do
The primary suffering in HFMD comes from oral pain. Children who are unable to eat or drink because of ulcer pain risk dehydration — the main medical concern in otherwise uncomplicated cases. Offering cool fluids, ice chips, or ice lollies can soothe the mouth and encourage fluid intake. Soft foods at room temperature — yogurt, mashed banana, smoothies — are better tolerated than acidic or salty foods which sting the ulcers.

Managing Fever and Discomfort
Fever in HFMD typically ranges from 38°C to 39.5°C and usually settles within two to three days. In homeopathic management, fever is understood as part of the immune response rather than an enemy to be suppressed, and remedies are selected to support the immune process rather than reduce the temperature for its own sake. Belladonna is indicated for high fever with flushed face and hot, dry skin in the early phase. Ferrum Phosphoricum suits the gentler fever of the second phase. Keeping the child cool, hydrated, and comfortable is more important than achieving a “normal” temperature by itself.
Paracetamol can be given alongside homeopathic remedies when fever is causing significant distress — there is no interaction between paracetamol and homeopathic remedies. The key clinical decision point is ensuring the child continues to take fluids — frequent small sips of cool water, diluted fruit juice, or oral rehydration solution.
When to See a Doctor Urgently
Most children with HFMD do not require medical assessment beyond reassurance. However, parents should seek urgent medical attention if the child has a very high fever (above 39.5°C) that does not respond to paracetamol, develops a stiff neck, severe headache, photophobia, difficulty breathing, persistent vomiting, unusual drowsiness, or a rash that looks like bruising or small red or purple spots (petechiae) — all of which may indicate a rare but serious complication requiring immediate assessment.
Signs of dehydration — reduced urine output, dry mouth, sunken eyes, and lethargy — also require medical assessment. Any infant under three months old with fever should be seen by a doctor promptly regardless of suspected cause. These red flags are not abrogated by homeopathic treatment — they are indications for immediate conventional assessment.
