Lambert-Eaton Myasthenic Syndrome (LEMS) is a rare autoimmune neuromuscular junction disorder in which antibodies mistakenly attack voltage-gated calcium channels at nerve endings, impairing the release of acetylcholine needed for normal muscle contraction. This produces proximal muscle weakness — most noticeable in the hips and thighs — along with autonomic symptoms such as dry mouth. Non-paraneoplastic LEMS, more common in younger patients and often associated with other autoimmune conditions, still requires this same thorough initial cancer screening, since it cannot be distinguished from paraneoplastic LEMS by symptoms alone.

Diagnosis and Essential Medical Treatment

Diagnosis combines the characteristic clinical pattern of proximal weakness with post-exercise facilitation, blood testing for voltage-gated calcium channel antibodies, and electromyography showing a distinctive incremental response pattern on repetitive nerve stimulation. Once diagnosed, management addresses two parallel tracks: thorough cancer screening and, if a tumour is found, oncological treatment (which often improves the neurological symptoms as well), and separately, symptomatic neuromuscular treatment with medications such as amifampridine (which improves nerve signal transmission) or immunosuppressive therapy for non-paraneoplastic cases. This is specialist neurological and oncological territory requiring close, ongoing follow-up, and any delay in cancer screening to pursue alternative treatment carries a serious risk of missing a treatable malignancy at an earlier, more favourable stage.
