A lipoma is a benign, slow-growing tumour composed of mature adipose (fat) tissue encapsulated in a thin fibrous capsule, arising in the subcutaneous tissue of the trunk, neck, shoulders, arms, and thighs. Lipomas are the most common soft tissue tumour in adults — affecting approximately 1 in 1000 individuals — and are generally harmless, presenting as a soft, mobile, non-tender subcutaneous lump. Multiple lipomas (lipomatosis) can occur in familial lipomatosis or conditions such as Madelung's disease. Constitutional homeopathy is one of the most sought-after alternatives to surgical removal for lipomas, with clinical experience suggesting measurable reduction in size and prevention of new lipoma formation.

When to Investigate a Lipoma

Most lipomas are diagnosed clinically — soft, mobile, lobulated, non-tender subcutaneous lump with a characteristic slipping sensation on palpation. Features warranting further investigation include: rapid growth (any lump that doubles in size over weeks rather than months), firmness or fixation to underlying structures, size greater than 5 cm, deep location (subfascial or intermuscular), tenderness, skin tethering, or overlying skin changes. These features raise concern for liposarcoma — a malignant soft tissue tumour — requiring urgent imaging (MRI) and surgical referral. Most typical soft tissue lipomas do not require investigation beyond clinical examination.

Surgery for Lipomas

Surgical excision of a lipoma is indicated for: rapidly growing lipomas, painful lipomas (often angiolipomas — a variant with a vascular component), lipomas causing functional impairment or pressure on nerves (e.g., neural lipomas causing tingling), cosmetically unacceptable lipomas, and lipomas for which malignancy cannot be excluded clinically. Liposuction aspiration is an alternative for soft lipomas without a firm capsule. For asymptomatic lipomas under 5 cm that are clinically typical, observation is acceptable — surgery is not necessary, and constitutional homeopathy offers a non-invasive alternative to reduce size and prevent recurrence.
