Orchitis — inflammation of one or both testes — presents with painful testicular swelling, redness, and systemic features of infection. It occurs most commonly as part of epididymo-orchitis (combined epididymis and testis inflammation, the most common form in adults), either from ascending sexually transmitted infections (Chlamydia trachomatis, Neisseria gonorrhoeae in under-35s) or urinary tract pathogens (E. coli, Klebsiella in over-35s). Viral orchitis — predominantly from mumps — occurs in approximately 30% of post-pubertal males with mumps and can cause testicular atrophy and reduced fertility. Acute orchitis requires proper investigation and antibiotic treatment; constitutional homeopathy is most valuable for the chronic, post-infective phase and for mumps orchitis to prevent testicular atrophy.

Excluding Testicular Torsion

Any sudden-onset unilateral testicular pain in a young man requires urgent exclusion of testicular torsion — twisting of the testis on the spermatic cord causing vascular occlusion and infarction. Testicular torsion is a surgical emergency — the testicle must be untwisted and fixed (orchidopexy) within 6 hours of symptom onset to preserve the organ. Cremasteric reflex is absent in torsion but usually present in epididymo-orchitis. Colour Doppler ultrasound differentiates torsion (absent flow) from orchitis (increased flow) — when clinical doubt exists, surgical exploration takes priority over waiting for imaging. Homeopathy has no role in acute testicular torsion.

Infectious Orchitis and STI Testing

In men under 35 with epididymo-orchitis, STI testing (urethral swab for Chlamydia, gonorrhoea) and empirical antibiotic treatment (doxycycline plus single-dose ceftriaxone) covers both pathogens pending results. Partner notification and treatment is required if STI is confirmed. In men over 35, midstream urine culture guides antibiotic choice. Mumps orchitis has no specific antiviral treatment — symptomatic management with scrotal support, analgesia, and bed rest. Bilateral mumps orchitis causes significant testicular atrophy and subfertility in up to 30% of cases — homeopathic intervention during the acute phase may reduce atrophy risk.

Constitutional Homeopathic Approach

Constitutional homeopathy for orchitis addresses the specific testicular inflammation pattern (acute vs chronic, unilateral vs bilateral), the associated features (epididymitis, varicocele, fertility concern, post-mumps atrophy), the pain character (dull aching, sharp, worse movement, better support), and the constitutional type. Homeopathy is most beneficial in the subacute and chronic phase after antibiotic treatment for infectious orchitis, and in acute mumps orchitis to reduce inflammation and prevent atrophy.
