Osteochondritis dissecans (OCD) is a joint condition in which a small segment of subchondral bone, along with its overlying cartilage, loses part of its blood supply. Over time this segment can soften, and in more advanced cases separate partially or completely from the surrounding healthy bone, sometimes forming a loose fragment within the joint. It occurs most often in active adolescents and young adults, typically at the knee, though the elbow and ankle are also common sites. The condition's outlook depends heavily on the patient's remaining growth potential and how stable the fragment is at diagnosis, which is why prompt orthopaedic evaluation and staged, imaging-guided management are central to good outcomes.
Understanding Osteochondritis Dissecans
The exact cause of OCD is not fully understood, but repetitive microtrauma to the joint surface, reduced local blood supply, and a genetic predisposition are all thought to contribute. It is most frequently seen in the knee, on the medial femoral condyle, but also occurs in the elbow (capitellum), often in young throwing or gymnastics athletes, and in the ankle (talar dome). Typical symptoms include activity-related joint pain and swelling that improves with rest, a vague ache that is difficult to localise precisely, and, once a fragment becomes unstable or detaches, mechanical symptoms such as catching, locking, or a sensation of the joint giving way. Any young athlete with persistent, unexplained joint pain — especially at the knee, elbow, or ankle — that does not settle with rest deserves orthopaedic assessment rather than being dismissed as routine growing pains.
Diagnosis and Conventional Management
X-rays can identify many OCD lesions, but MRI is the key investigation for staging lesion stability and planning treatment, since management differs substantially between stable and unstable fragments. Stable lesions in skeletally immature patients are frequently managed non-surgically, with activity modification, protected weight-bearing, and physiotherapy, and have a good capacity to heal given the remaining growth potential. Unstable lesions, loose fragments, or lesions in patients who are skeletally mature typically require surgical treatment — options range from drilling to stimulate healing, to fixation of the fragment, to cartilage restoration procedures for larger defects. Ongoing orthopaedic follow-up with repeat imaging is standard practice to confirm healing and guide safe return to sport.
