Diagnosis relies on skin or mucosal biopsy with immunofluorescence studies and specific antibody testing, combined with a thorough search for the underlying tumour once the diagnosis is suspected — this typically involves imaging and haematology-oncology evaluation. Treating the underlying malignancy, where one is found and treatable (for example, surgical removal of a thymoma, or treatment of an underlying lymphoma), is a central part of management and can improve the skin and mucosal disease. Alongside this, immunosuppressive therapy — corticosteroids and often additional agents such as rituximab — is used to control the autoimmune blistering. Given the combination of severe mucosal disease, an underlying cancer, and potential lung involvement, this condition requires coordinated care between dermatology, haematology-oncology, and pulmonology.
