Reactive arthritis — formerly called Reiter's syndrome — is an inflammatory arthritis triggered by an extra-articular infection, typically genitourinary (Chlamydia trachomatis) or gastrointestinal (Salmonella, Shigella, Campylobacter, Yersinia). The classic triad of arthritis, conjunctivitis, and urethritis (remembered as 'can't see, can't pee, can't bend the knee') occurs in a minority of patients — most present with asymmetric lower limb oligoarthritis developing 1 to 4 weeks after the triggering infection. HLA-B27 is positive in 60 to 80% of patients and associated with more severe, persistent disease. Understanding Reactive Arthritis

Reactive arthritis typically affects large weight-bearing joints of the lower limb — knee, ankle, subtalar joint — with asymmetric involvement. Extra-articular features include: conjunctivitis or uveitis, urethritis or cervicitis, mouth ulcers, circinate balanitis (annular erosions on the glans penis), keratoderma blennorrhagica (hyperkeratotic skin lesions on palms and soles resembling pustular psoriasis), and enthesitis (Achilles tendon insertion, plantar fascia). Investigations: STI testing (urethral/cervical swab for Chlamydia, gonorrhoea), stool culture if recent diarrhoea, HLA-B27, CRP/ESR, and baseline joint X-rays. Most cases resolve within 6 months — 20% develop chronic arthritis.

Treating the Triggering Infection

If the triggering infection is still active or recent, antibiotic treatment is indicated: doxycycline 100mg twice daily for 3 months for Chlamydia-triggered reactive arthritis (prolonged treatment may reduce chronicity), appropriate antibiotics for confirmed gastrointestinal pathogens. Partner notification and treatment is required for Chlamydia-triggered cases. The arthritis itself is treated with NSAIDs for mild disease, intra-articular corticosteroid injections for persistent joint inflammation, and sulfasalazine or methotrexate for chronic, refractory reactive arthritis. Biological agents (TNF inhibitors) are used for severe HLA-B27-positive disease.
