Spinal Intradural Arachnoid Cysts

Dr. Meera ThakurAugust 20267 min read

Spinal intradural arachnoid cysts are fluid-filled sacs that form within the arachnoid membrane — one of the three protective layers surrounding the spinal cord — inside the dural sac. They may be congenital, arising from a developmental split in the arachnoid layer, or acquired following trauma, infection, or spinal surgery that triggers scarring and abnormal cerebrospinal fluid pockets. Many are found incidentally on imaging and never cause problems, but those that enlarge can compress the spinal cord or nerve roots, producing progressive neurological symptoms that require neurosurgical evaluation to determine whether treatment is needed.

How These Cysts Present

Symptoms, when present, develop gradually as the cyst enlarges and encroaches on available space within the spinal canal — back pain, progressive leg weakness or stiffness, sensory changes, and, less commonly, bladder or bowel dysfunction if the cyst compresses the lower spinal cord or nerve roots (the cauda equina). Symptoms sometimes fluctuate with posture or activity because cerebrospinal fluid dynamics within the cyst can change with position, a distinctive clue that can help differentiate arachnoid cysts from other causes of cord compression. Congenital cysts are sometimes first noticed in childhood or young adulthood, while acquired cysts typically develop months to years after the inciting trauma, infection, or surgery.

Diagnosis

MRI is the investigation of choice, clearly showing the fluid-filled cyst, its exact location relative to the spinal cord, and the degree of any cord displacement or compression. Because arachnoid cysts have imaging characteristics that can overlap with other spinal cysts (such as those covered elsewhere on this site), a specialist radiological and neurosurgical assessment is needed to confirm the diagnosis and, where relevant, clarify whether the cyst is congenital or related to a prior event such as surgery or trauma.

Standard Neurosurgical Management

Asymptomatic or incidentally found cysts that are not causing cord compression are generally monitored with periodic clinical review and imaging rather than treated surgically, since surgery carries its own risks and many cysts remain stable indefinitely. Symptomatic cysts causing progressive neurological deficit are typically treated surgically — options include cyst fenestration (opening the cyst wall to allow fluid to disperse), partial or complete cyst wall excision, or, less commonly, a shunting procedure — chosen according to the cyst's size, location, and the surgeon's assessment. Outcomes are generally favourable when surgery is performed before significant, established neurological deficit has developed.

Where Constitutional Homeopathy Fits In

Homeopathy cannot shrink or dissolve an arachnoid cyst or reverse spinal cord compression that has already caused neurological deficit — these require the structural neurosurgical assessment described above. Constitutional treatment is offered as supportive, adjunct care: for pain, general wellbeing, and recovery around any surgical treatment, and for reassurance and comfort while an asymptomatic or stable cyst is being monitored. Any new or worsening neurological symptom should prompt urgent reassessment by the treating neurosurgical team rather than being managed with homeopathy alone.

Key Points at a Glance

  • Spinal intradural arachnoid cysts may be congenital or follow trauma, infection, or spinal surgery

  • Symptoms can fluctuate with posture as cerebrospinal fluid dynamics within the cyst change

  • MRI confirms the diagnosis and clarifies the cyst's relationship to the spinal cord

  • Asymptomatic cysts are typically monitored; symptomatic, compressive cysts are treated surgically

  • Homeopathy offers supportive care only — new or worsening neurological symptoms need urgent neurosurgical reassessment
