Thoracic outlet syndrome (TOS) results from compression of the nerves and blood vessels — the brachial plexus, subclavian artery, and subclavian vein — as they pass through the narrow space between the collarbone and the first rib. Unlike many of the rare syndromes on this site, TOS is a relatively common condition, seen often in people with repetitive overhead activity, poor posture, or an anatomical variant such as an extra cervical rib. Understanding Thoracic Outlet Syndrome

TOS is classified into three types based on which structure is compressed. Neurogenic TOS, affecting the brachial plexus nerves, accounts for roughly 90% of cases and is by far the most common. Venous TOS compresses the subclavian vein, sometimes causing dramatic arm swelling, and arterial TOS compresses the subclavian artery — the rarest but most serious type, as it can affect blood flow to the arm. Common contributing factors include a cervical rib or other bony anomaly, poor posture with rounded shoulders, repetitive overhead work or sport (swimming, throwing, weightlifting), and, less commonly, trauma such as a whiplash injury.

Recognising the Symptoms

Neurogenic TOS typically causes pain, numbness, and tingling in the arm and hand, often most noticeable in the ring and little fingers, along with a feeling of weakness or heaviness that worsens with overhead activity or carrying weight. Venous TOS presents with arm swelling, heaviness, and a bluish discoloration, sometimes appearing suddenly after vigorous arm use (effort thrombosis). Arterial TOS is less common but more concerning, presenting with coldness, pallor, and a diminished pulse in the affected arm — symptoms that warrant urgent vascular assessment.

Diagnosis and Conventional Management

Diagnosis combines a careful clinical examination with provocative tests (such as the Adson's or Roos test, which reproduce symptoms in specific arm positions), supported by imaging — X-ray to check for a cervical rib, and CT or MR angiography when a vascular cause is suspected. For the common neurogenic type, physiotherapy focused on postural correction and nerve mobility is the established first-line treatment, and most people improve without surgery. Surgical decompression (removing a cervical rib or the first rib) is reserved for cases that do not respond to conservative treatment, or for the vascular types, where it is more often needed.
