Idiopathic Oedema and PMS Fluid Retention
Idiopathic oedema — occurring predominantly in women of reproductive age — causes cyclical or non-cyclical fluid retention without identifiable cardiac, renal, or hepatic cause. Weight may fluctuate by 2 to 4 kg within a day. Contributing factors include upright posture (venous pooling), increased capillary permeability, and reduced lymphatic function. Premenstrual oedema is driven by progesterone-induced aldosterone activation and prostaglandin E2 causing capillary permeability in the luteal phase. Reducing dietary sodium, increasing potassium intake, limiting alcohol, and avoiding tight clothing improve idiopathic oedema. Diuretics are often overused for idiopathic oedema and can paradoxically worsen it by activating the renin-angiotensin system.
