Chronic obstructive pulmonary disease (COPD) — a progressive, irreversible airflow limitation caused by tobacco smoking and biomass fuel exposure — is the third leading cause of death globally. Characterised by emphysema (alveolar destruction) and chronic bronchitis (productive cough for 3 months in two consecutive years), COPD produces progressive breathlessness, exercise limitation, and acute exacerbations that accelerate lung function decline. COPD cannot be cured; however, constitutional homeopathy significantly improves quality of life, reduces exacerbation frequency, and reduces breathlessness severity as an adjunct to pulmonologist-directed management.

Understanding COPD

COPD is staged by FEV1 (forced expiratory volume in 1 second) on spirometry — GOLD 1 (mild: FEV1 ≥80% predicted) to GOLD 4 (very severe: FEV1 <30%). Symptoms include progressive breathlessness on exertion, chronic productive cough with mucoid sputum, wheeze, chest tightness, and frequent respiratory infections. Complications include pulmonary hypertension, cor pulmonale (right heart failure), polycythaemia (from chronic hypoxaemia), and pneumothorax. Long-term oxygen therapy (LTOT) for resting hypoxaemia (SpO2 ≤88%) is the only intervention proven to improve survival in COPD.

What Cannot Be Replaced in COPD Management

Smoking cessation is the single most important intervention in COPD — it slows FEV1 decline and reduces exacerbation risk, regardless of disease stage. Long-acting bronchodilators (LABA, LAMA) significantly reduce breathlessness, improve exercise tolerance, and reduce exacerbations — they must not be discontinued. Pulmonary rehabilitation — structured exercise training and education — is the most evidence-based non-pharmacological intervention. LTOT for hypoxaemia improves survival and must not be replaced. Acute COPD exacerbations with purulent sputum and increased breathlessness require systemic corticosteroids and antibiotics — not homeopathy alone.

Constitutional Homeopathic Approach

Constitutional homeopathy for COPD addresses the breathlessness character (paroxysmal, positional, exertional, nocturnal), the sputum type (mucoid, purulent, copious, difficult to raise), the cough character (loose, rattling, dry, hacking), the thermal state, the anxiety associated with breathlessness, and the constitutional type. Treatment over 4 to 8 weeks improves sputum clearance, reduces breathlessness at rest and with mild exertion, and extends the inter-exacerbation interval. It does not reverse the underlying FEV1 decline.

Antimonium Tartaricum suits advanced COPD with rattling, copious mucus in the chest that cannot be raised — the patient is too weak to cough and may require postural drainage. Arsenicum Album addresses COPD with nocturnal breathlessness, burning chest constriction, intense anxiety at night, and the prostrated, chilly patient better from sitting up and warmth. Stannum Metallicum suits COPD with profuse, greenish, sweet-tasting sputum raised with effort — the chest feels empty and the patient is extremely weak. Lycopodium addresses COPD with right-sided chest predominance, flaring nostrils, and fan-like nasal movement during breathing.
