Impetigo — a highly contagious superficial bacterial skin infection caused predominantly by Staphylococcus aureus (and less commonly Streptococcus pyogenes) — is the most common bacterial skin infection in children, presenting with honey-coloured crusted sores around the mouth, nose, and exposed skin areas. It spreads by direct contact and is associated with overcrowding, poor hygiene, hot humid climates, and skin trauma (insect bites, eczema, abrasions). Antibiotic treatment — topical mupirocin or fusidic acid for localised disease, oral antibiotics (flucloxacillin, amoxicillin-clavulanate) for extensive or bullous impetigo — is required for bacterial clearance. Constitutional homeopathy supports healing, reduces recurrence, and addresses the susceptible constitution.

Types of Impetigo

Non-bullous (crusted) impetigo — the most common form (70%) — begins as vesicles and pustules that rupture, leaving the classic honey-coloured adherent crusts. Bullous impetigo — caused specifically by S. aureus toxin-producing strains — produces large, fragile blisters (bullae) filled with clear or turbid fluid that rupture to leave a shallow erosion with a peripheral scale, without the classical honey crust. Ecthyma is a deeper form penetrating to the dermis, leaving a punched-out ulcer with a grey-yellow crust — more common in immunocompromised patients and those with poor hygiene. Rarely, Streptococcal impetigo can be complicated by post-streptococcal glomerulonephritis — requiring urine monitoring.

Antibiotic Treatment Is Required

Impetigo requires antibiotic treatment to achieve bacterial clearance and reduce transmission — particularly in school-age children who can spread infection to classmates. Topical mupirocin or fusidic acid is effective for localised, non-bullous impetigo. Oral antibiotics (flucloxacillin first-line in MRSA-negative settings, co-amoxiclav as second-line) are required for bullous impetigo, extensive disease, systemic features (fever, lymphadenopathy), or immunocompromise. School exclusion until 48 hours after starting antibiotics and until all sores are crusted and dry is standard infection control. Homeopathy does not clear established bacterial skin infection and must not be offered as an alternative to antibiotics.

Constitutional Homeopathic Approach

Constitutional homeopathy for impetigo addresses the susceptible constitution (children with eczema, nutritional deficiency, iron deficiency anaemia, or immunodeficiency who have recurrent impetigo), the specific lesion character (crusted, bullous, ecthymatous), the distribution, the thermal and moisture modalities, and the constitutional type. Treatment as an adjunct reduces healing time, prevents secondary spread, supports the skin barrier, and addresses the recurrent susceptibility after completing antibiotic treatment.

Antimonium Crudum addresses impetigo with thick, honey-coloured crusts, particularly on the face and scalp — the irritable, tongue-coated, over-eating, warm-bathing-aggravated child who resists examination. Graphites suits impetigo with sticky, honey-like oozing from eczematous skin — the chilly, constipated, melancholic constitutional type with slow skin healing. Mezereum addresses impetigo on the scalp with thick, leather-like crusts from which pus oozes and mats the hair — severe, ulcerated crusting. Hepar Sulphuricum suits impetigo prone to secondary infection — the extremely sensitive, chilly, irritable patient where every wound suppurates.
