BHMS — Classical Homeopathic Physician

Overactive bladder (OAB) — defined as urinary urgency (a sudden compelling desire to void that is difficult to defer) with or without urgency incontinence, usually accompanied by frequency (8 or more voids per day) and nocturia (2 or more voids per night) — affects an estimated 12% of adults, with significant impact on quality of life, sleep, and emotional wellbeing. OAB results from detrusor overactivity (involuntary bladder contractions) with or without a neurological cause. Urinary tract infection, bladder cancer, bladder stones, and interstitial cystitis must be excluded before diagnosing idiopathic OAB. Constitutional homeopathy effectively reduces detrusor overactivity and improves bladder capacity.

Excluding Serious Causes

Urinary urgency and frequency have important differential diagnoses that must be excluded before attributing symptoms to OAB: urinary tract infection (dipstick and MSU culture), bladder cancer (haematuria — visible or non-visible — with urgency requires urgent urology referral and cystoscopy), interstitial cystitis / bladder pain syndrome (pelvic pain, suprapubic pressure, frequency without incontinence), bladder stones (typically haematuria with urgency), and neurological causes (multiple sclerosis, Parkinson's disease, spinal cord injury — neurogenic detrusor overactivity). Prostate enlargement in men causes mixed symptoms including urgency and frequency.

Behavioural and First-Line Interventions

Bladder training — scheduled voiding with progressive lengthening of the voiding interval — is the most effective first-line non-pharmacological treatment. Fluid intake optimisation (1.5 to 2 litres per day — avoiding caffeinated drinks and alcohol which are bladder irritants), caffeine reduction, and pelvic floor muscle training are evidence-based lifestyle interventions. Antimuscarinics (oxybutynin, solifenacin, tolterodine) and beta-3 agonists (mirabegron) are first-line pharmacological treatment — effective but associated with dry mouth, constipation, and cognitive effects (particularly oxybutynin in the elderly). Botulinum toxin injection into the detrusor is a third-line option for refractory OAB.

Constitutional Homeopathic Approach

Constitutional homeopathy for OAB addresses the urgency character (sudden, irresistible, before reaching the toilet; or gradual, building urgency), the frequency pattern (day vs night predominant), the incontinence type (urgency, stress, or mixed), associated features (burning, pain, haematuria — these suggest UTI or interstitial cystitis rather than OAB), and the constitutional type. The thermal, emotional, and constitutional picture guides remedy selection. Treatment over 6 to 12 weeks progressively reduces urgency episodes and voiding frequency, improving bladder functional capacity.

Key Remedies

Causticum addresses OAB with urgency incontinence on coughing, sneezing, or laughing (stress component), night-time urgency, and the chilly, arthritic, sympathetic constitutional type. Equisetum Hyemale suits frequent, urgent urination with a constant sensation of fullness — the bladder that never feels empty — without infection or structural cause. Kreosotum addresses severe urgency incontinence where the patient cannot reach the toilet in time — must void immediately on the urge, wets before reaching the toilet, worse at night. Gelsemium suits OAB from nervous anxiety — the patient with frequent urination from anticipation, examination, and performance anxiety.

Key Points at a Glance

  • Haematuria with urgency requires urgent urology referral for cystoscopy — exclude bladder cancer

  • Bladder training (scheduled voiding with interval extension) is the most evidence-based first-line treatment

  • Caffeine and alcohol are bladder irritants — reducing them significantly improves OAB symptoms

  • Kreosotum suits severe urgency incontinence — cannot reach the toilet in time; worse at night

  • Causticum suits OAB with stress incontinence component in the chilly, arthritic constitutional type
