Haemorrhoids — piles — are dilated venous cushions in the anorectal canal that become symptomatic when congested, prolapsed, or thrombosed. Internal haemorrhoids above the dentate line are painless but bleed; external haemorrhoids below it cause pain. Constitutional homeopathy offers meaningful relief for all grades of symptomatic haemorrhoids and addresses the underlying constipation and venous congestion that drives recurrence — often avoiding the need for procedural intervention in early grades.

Why Piles Develop

Haemorrhoids develop from chronic straining at stool, constipation, prolonged sitting, low dietary fibre, pregnancy, and portal hypertension. Straining raises intra-abdominal pressure and engorges the haemorrhoidal venous plexus over time. Pregnancy causes direct compression of the inferior vena cava and pelvic veins in addition to hormonal smooth muscle relaxation. Dietary factors — insufficient fibre, inadequate fluid intake, and excessive red chilli consumption — are the most modifiable contributing factors in the Indian context and must be addressed concurrently with any treatment.

Grades and Symptoms

Internal haemorrhoids are graded I to IV based on prolapse: Grade I bleeds without prolapse; Grade II prolapses on straining but reduces spontaneously; Grade III requires manual reduction; Grade IV is permanently prolapsed and cannot be reduced. Painless bright red rectal bleeding after defaecation is the hallmark of internal haemorrhoids. External haemorrhoids and thrombosed haemorrhoids produce severe pain. Any rectal bleeding in a patient over 40 should be evaluated by colonoscopy to exclude colorectal malignancy before attributing it to haemorrhoids.

Constitutional Homeopathic Treatment

Constitutional homeopathy for haemorrhoids addresses the venous congestion, the specific character of bleeding and pain, the bowel habit, and the complete constitutional picture. Improvement is typically seen within 4 to 8 weeks for acute symptoms, with continued treatment reducing recurrence. Concurrent attention to dietary fibre, fluid intake, and avoiding prolonged sitting is essential — no remedy substitutes for these measures. Grade III and IV prolapsed haemorrhoids with heavy bleeding may require procedural intervention alongside homeopathic treatment.

Key Remedies

Aesculus Hippocastanum is the foremost pile remedy — it addresses the engorged, purple, protruding haemorrhoids with a sensation of small sticks in the rectum, backache, and venous congestion. Nux Vomica suits piles from constipation and sedentary lifestyle with constant ineffectual urging and partial unsatisfying stools. Hamamelis Virginiana addresses profuse, dark, passive haemorrhoidal bleeding with venous soreness. Muriatic Acid suits the most sensitive, touch-averse haemorrhoids that are too tender to be touched during examination.
