Raynaud's phenomenon — episodic digital vasospasm triggered by cold or emotional stress, causing a characteristic triphasic colour change in the fingers and toes: white (pallor, ischaemia) → blue (cyanosis, hypoxia) → red (rubor, reactive hyperaemia) — affects approximately 3 to 5% of the general population. Primary Raynaud's (Raynaud's disease) occurs without an underlying cause, predominantly in young women, and has a benign course. Secondary Raynaud's occurs in the context of connective tissue disease (particularly systemic sclerosis/scleroderma, SLE, rheumatoid arthritis, polymyositis), occupational vibration exposure, or medication effects (beta-blockers, ergotamines, bleomycin). Constitutional homeopathy is highly effective for primary Raynaud's and supports secondary Raynaud's alongside treatment of the underlying condition.

Primary vs Secondary Raynaud's

Distinguishing primary from secondary Raynaud's is clinically important. Primary Raynaud's: young women, bilateral, symmetrical attacks, mild digital colour change, no digital pitting or ulceration, no other systemic symptoms, normal nailfold capillaroscopy. Secondary Raynaud's: older onset, asymmetric or severe attacks, digital ulceration, skin thickening, other autoimmune features, abnormal nailfold capillaroscopy (showing dilated, tortuous, or absent capillaries). ANA and autoimmune screen should be performed in all patients — particularly with late onset, asymmetric attacks, or systemic features. Secondary Raynaud's requires specialist rheumatology assessment and management of the underlying condition.

Management of Raynaud's

Lifestyle measures are first-line: avoiding cold exposure (gloves, layered clothing, hand warmers), avoiding cigarettes (vasoconstriction), reviewing vasoconstrictive medications (beta-blockers), stress management. Calcium channel blockers (nifedipine) are first-line pharmacological treatment for moderate-severe primary Raynaud's — reducing vasospasm frequency and severity. Phosphodiesterase-5 inhibitors (sildenafil) and IV prostacyclin infusions are used for severe secondary Raynaud's with digital ischaemia. Digital sympathectomy is a last resort for critical ischaemia. Constitutional homeopathy is highly effective for primary Raynaud's — often achieving complete remission without the headache and ankle oedema side effects of nifedipine.
