BHMS — Classical Homeopathic Physician

Scleroderma — systemic sclerosis (SSc) — is a systemic autoimmune disease characterised by progressive fibrosis of the skin and internal organs, microvascular injury, and immune dysregulation. SSc is divided into limited cutaneous SSc (lcSSc — formerly CREST syndrome: Calcinosis, Raynaud's, oEsophageal dysmotility, Sclerodactyly, Telangiectasia; associated with anti-centromere antibody) and diffuse cutaneous SSc (dcSSc — rapidly progressive skin thickening proximal to elbows, associated with anti-Scl-70 antibody and higher risk of pulmonary fibrosis, renal crisis). SSc is a serious multi-system disease requiring specialist rheumatology care. Constitutional homeopathy supports skin softening, improves Raynaud's severity, and enhances quality of life as an adjunct.

Recognising the Seriousness of SSc

Systemic sclerosis is associated with significant morbidity and mortality from pulmonary arterial hypertension (PAH), interstitial lung disease (ILD), scleroderma renal crisis (SRC — rapidly progressive hypertension and renal failure requiring urgent ACE inhibitor treatment), oesophageal and bowel dysmotility, and cardiac involvement. Annual screening for PAH (echocardiography), ILD (HRCT, pulmonary function tests), and renal function is mandatory. Scleroderma renal crisis is a medical emergency — presenting with severe hypertension and rapidly rising creatinine — requiring immediate ACE inhibitor treatment (captopril). Homeopathy cannot reverse established organ fibrosis and must not substitute specialist monitoring and disease-modifying treatment.

Specialist Management

Disease-modifying treatment in SSc: methotrexate for early diffuse skin disease, mycophenolate mofetil (MMF) and nintedanib for ILD, endothelin receptor antagonists (bosentan, ambrisentan) and phosphodiesterase-5 inhibitors (sildenafil) for PAH and Raynaud's. Proton pump inhibitors for oesophageal reflux, prokinetics for gut dysmotility. Autologous haematopoietic stem cell transplantation (HSCT) is an option for rapidly progressive severe dcSSc. Annual echocardiography, 6-monthly PFTs, and renal monitoring are non-negotiable. Patient education — vasospasm trigger avoidance, ulcer wound care — is critically important.

Constitutional Homeopathic Approach

Constitutional homeopathy for SSc addresses the skin thickening and sclerodactyly (constitution, extent, rate of progression), the Raynaud's severity and digital ulceration, the gastrointestinal involvement (dysphagia, reflux, bloating), the emotional picture (grief, loss of self-image, profound anxiety about prognosis), and the constitutional type. Homeopathic treatment as an adjunct over 6 to 12 months supports skin softening in early disease, significantly reduces Raynaud's frequency and severity, and improves emotional wellbeing and quality of life alongside specialist care.

Key Remedies

Causticum addresses SSc with progressive skin contraction and tightening — the chilly, sympathetic, justice-sensitive constitutional type with contraction tendency and functional limitation. Phosphorus suits SSc in the tall, lean, haemorrhagic, warm-blooded constitutional type — telangiectasia, thin skin, and the sensitive, sociable patient with ILD. Arsenicum Album addresses SSc with intense burning, restless anxiety, fastidiousness, and the extreme physical debility pattern — worse at night and from cold. Silicea addresses SSc in the cold, timid, weak-assimilation constitutional type with tendency to suppuration and induration of connective tissue.

Key Points at a Glance

  • Scleroderma renal crisis is a medical emergency — severe hypertension with rising creatinine requires immediate ACE inhibitor treatment

  • Annual echocardiography for PAH screening is mandatory in all SSc patients

  • Homeopathy cannot reverse organ fibrosis — it is a quality-of-life adjunct alongside specialist care

  • Causticum suits progressive skin contraction; Phosphorus suits ILD-associated SSc in tall, haemorrhagic type

  • Raynaud's severity can be significantly reduced with constitutional homeopathy — improving digital circulation
