A chalazion — a meibomian gland cyst — is a chronic, sterile inflammatory granuloma of the eyelid caused by obstruction and retention of the lipid secretion of the meibomian glands (sebaceous glands within the tarsal plates of the eyelids). Unlike a stye (hordeolum), which is an acute bacterial infection presenting with pain, redness, and a pointed pustule, a chalazion is typically painless, round, and firm — developing over days to weeks after an initial stye that fails to resolve. Constitutional homeopathy is highly effective for chalazion resolution, avoiding the need for surgical incision and curettage in most cases.

Chalazion vs Stye — The Distinction Matters

An external hordeolum (stye) is an acute staphylococcal infection of a Zeis or Moll gland at the eyelid margin — painful, pointing externally. An internal hordeolum is a meibomian gland infection — more painful, pointing internally. A chalazion develops when a hordeolum fails to drain and the lipid contents provoke a foreign body granulomatous response. Chalazia are usually painless unless secondarily infected. Blepharitis (chronic eyelid margin inflammation from meibomian gland dysfunction) predisposes to recurrent chalazia — treating blepharitis reduces recurrence. Recurrent chalazia in middle-aged or older patients require biopsy to exclude sebaceous gland carcinoma, which can masquerade as a chalazion.

When Surgery Is Indicated

Small chalazia often resolve spontaneously within a few months. Warm compresses applied for 5 to 10 minutes four times daily soften the retained secretion and promote drainage — this is the first-line treatment. Large chalazia distorting the cornea and causing astigmatism, or persistent chalazia causing cosmetic concern after 4 to 6 months, may require incision and curettage (I&C) under local anaesthesia. Intralesional triamcinolone injection is an alternative to surgery for large chalazia.
